This is legit. Those blank spots appear at first but they pass after a few minutes and the original tattoo shows through again, albeit somewhat faded. Shortly afterwards the skin will become blistered and painful, just like a burn injury (which really is exactly what it is).
After a month or so, the skin should be healed and ready for another treatment. Tattoo removal usually takes between 6 and 12 treatments, depending on the complexity and density of the tattoo. Black ink is easier but more painful to remove, as a more focused laser is used. Bright colours may never be fully removed, but laser treatment is recommended to fade them before having the design covered with a new tattoo.
Source: I've had this done. Incidentally, make sure you pick your tattoos carefully. Don't think of laser treatment as an option, it hurts 10x worse than the tattoo itself ever does.

There's also the matter of the skin tone after the laser has made a pass: if this is simply a tattoo in reverse, why does the freshly cleared skin look different to the rest of the arm?

No it's not. The light is way larger than the detail of the sections it's "adding" in. This is removal.
